Summary
Sulfate-based bowel cleansing causes fluid and electrolyte losses, which with an ACE inhibitor increases the risk of acute kidney injury and hypotension. Electrolyte disturbances may also contribute to arrhythmia.
What this interaction means in plain words
Benazepril and Sodium sulfate / potassium sulfate / magnesium sulfate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Volume depletion from the bowel preparation reduces renal perfusion, which is poorly tolerated during ACE inhibition.
What to do
Encourage adequate fluid intake before and after the preparation and consider holding benazepril on the day of use in at-risk patients. Check renal function and electrolytes if the patient is elderly or has kidney disease.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
