Summary

Adding an ACE inhibitor to a regimen already containing the angiotensin receptor blocker valsartan produces dual renin-angiotensin system blockade. This raises the risk of symptomatic hypotension, hyperkalemia and acute kidney injury without a meaningful cardiovascular benefit in most patients.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Benazepril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Benazepril belongs to Cardiovascular.

Evidence level: High

Why it happens

Additive pharmacodynamic inhibition of the renin-angiotensin-aldosterone system at two different points, reducing glomerular filtration pressure and aldosterone-mediated potassium excretion.

What to do

Avoid the combination and use a single renin-angiotensin agent. If it is unavoidable, monitor blood pressure, serum potassium and creatinine closely.

What to ask your pharmacist
