Summary

Indomethacin can blunt the diuretic and antihypertensive effects of thiazides and promote sodium and fluid retention. In susceptible patients such as the elderly or those with volume depletion, the combination can also increase the risk of acute kidney injury.

What this interaction means in plain words

Bendroflumethiazide and Indomethacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

NSAIDs inhibit renal prostaglandin synthesis, reducing renal blood flow and sodium excretion, which opposes thiazide action.

What to do

Monitor blood pressure, fluid status, weight and renal function. Use the lowest effective indomethacin dose for the shortest time.

What to ask your pharmacist
