Summary
Salbutamol shifts potassium into cells and thiazide diuretics promote urinary potassium loss, so together they can produce clinically significant hypokalemia. This is most relevant with high or repeated salbutamol doses and can predispose to arrhythmias.
What this interaction means in plain words
Bendroflumethiazide and Ipratropium-salbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive hypokalemia from beta-2 agonist-driven intracellular potassium shift and thiazide-induced renal potassium wasting.
What to do
Monitor serum potassium, especially during acute asthma or COPD exacerbations or with high-dose nebulized therapy. Replace potassium as needed and check magnesium.
What to ask your pharmacist
