Summary
Ibutilide markedly prolongs the QT interval and carries a risk of torsades de pointes, and salbutamol can add to this through QT effects and hypokalemia. Hypokalemia and beta-agonist-induced tachycardia may increase proarrhythmic risk during and after ibutilide infusion.
What this interaction means in plain words
Ibutilide and Ipratropium-salbutamol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ibutilide belongs to Cardiovascular; Ipratropium-salbutamol belongs to Respiratory.
Why it happens
Additive QT prolongation, with beta-agonist-induced hypokalemia increasing torsades risk.
What to do
Correct potassium and magnesium before ibutilide and avoid unnecessary salbutamol dosing around the infusion. Monitor ECG continuously during and after administration.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
