Summary
Quinidine strongly inhibits CYP2D6, which reduces conversion of hydrocodone to hydromorphone, a minor active metabolite. Because hydrocodone is itself active, analgesia is usually preserved, but metabolism shifts toward CYP3A4 and hydrocodone levels may rise, and the effect on opioid response is unpredictable.
What this interaction means in plain words
Benzhydrocodone / acetaminophen and Quinidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Quinidine inhibits CYP2D6-mediated O-demethylation of hydrocodone, altering the parent-to-metabolite ratio.
What to do
Monitor for changes in analgesia and for sedation or respiratory depression, and adjust the opioid dose accordingly.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
