Summary

Both drugs prolong cardiac repolarization, and concurrent use raises the risk of torsades de pointes and sudden death. Quinidine may also reduce renal clearance of dofetilide, increasing its exposure.

What this interaction means in plain words

Dofetilide and Quinidine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Dofetilide belongs to Cardiovascular; Quinidine belongs to Anti Parasitic.

Evidence level: Moderate

Why it happens

Additive potassium channel blockade causing QT prolongation, with possible reduced renal elimination of dofetilide.

What to do

Do not coadminister; allow adequate washout of the first antiarrhythmic before starting the other, under ECG monitoring.

What to ask your pharmacist
