Summary

Thioridazine causes marked, dose-related QT prolongation and carries a boxed warning against co-use with other QT-prolonging drugs. Adding ofloxacin further increases the risk of torsades de pointes and sudden cardiac death.

What this interaction means in plain words

Ofloxacin and Thioridazine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Ofloxacin belongs to Anti Bacterial; Thioridazine belongs to Central Nervous System.

Evidence level: High

Why it happens

Additive blockade of cardiac potassium channels on top of the strong QT-prolonging effect of thioridazine.

What to do

Avoid the combination and select an antibiotic without QT effects. If the antibiotic cannot be changed, withhold thioridazine under specialist guidance.
