Summary
Echothiophate is an irreversible cholinesterase inhibitor that raises acetylcholine levels, which can add to the direct muscarinic stimulation of bethanechol. Although applied to the eye, systemic absorption can occur and cholinergic toxicity such as bradycardia, bronchospasm and gastrointestinal hypermotility is possible.
What this interaction means in plain words
Bethanechol and Echothiophate iodide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive cholinergic stimulation: direct muscarinic agonism plus inhibition of acetylcholine breakdown.
What to do
Monitor for systemic cholinergic effects, especially heart rate and respiratory status. Reduce or stop bethanechol if toxicity develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
