Overview
Beta-blockers may mask adrenergic symptoms of hypoglycemia and can enhance hypoglycemic effects of antidiabetic agents; DPP-4 inhibitors have low risk but recognition of hypoglycemia may be impaired.
Mechanism of action
Pharmacodynamic: beta-adrenergic blockade reduces catecholamine-mediated glycogenolysis and masks tachycardia/tremor, potentially worsening or obscuring hypoglycemia.
Clinical management
Educate patient; increase glucose monitoring when starting or changing doses; consider adjusting antidiabetic therapy if recurrent hypoglycemia; emphasize neuroglycopenic symptoms; note cardioselectivity does not eliminate risk.
