For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Additive blood pressure–lowering and orthostatic effects may increase risk of dizziness or syncope, particularly at initiation or dose escalation.

Mechanism of action

Pharmacodynamic: beta-1 blockade reduces heart rate/cardiac output; alpha-1A blockade causes vasodilation and decreased peripheral resistance; combined effects impair orthostatic compensation. No meaningful pharmacokinetic interaction expected.

Clinical management

If used together, ensure one agent is stabilized before starting the other; initiate tamsulosin at 0.4 mg and consider bedtime dosing; monitor seated and standing BP/HR at baseline and after changes; counsel on orthostatic precautions; reduce antihypertensive dose if symptomatic hypotension occurs.
