Summary
Blinatumomab causes a transient cytokine release that can suppress CYP450 enzymes. This may temporarily raise concentrations of CYP substrates, but clonidine is only partly metabolized by CYP2D6 and has a wide margin of safety.
What this interaction means in plain words
Blinatumomab and Clonidine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Blinatumomab belongs to Oncology; Clonidine belongs to Cardiovascular.
Why it happens
Cytokine-mediated transient suppression of hepatic CYP450 activity after blinatumomab infusion starts.
What to do
Monitor blood pressure and heart rate during the first days of each blinatumomab cycle and reduce clonidine if hypotension or bradycardia occurs.
What to ask your pharmacist
- Is there anything I should notice while taking both?
- Do I need to tell my doctor at the next visit?
