Summary
Blinatumomab causes a transient cytokine release at treatment start that can suppress CYP450 enzymes. Theophylline, a narrow therapeutic index CYP1A2 substrate, may accumulate and cause toxicity such as tachyarrhythmia, nausea or seizures.
What this interaction means in plain words
Blinatumomab and Theophylline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Blinatumomab belongs to Oncology; Theophylline belongs to Respiratory.
Why it happens
Cytokine-mediated transient suppression of hepatic CYP450 enzymes (including CYP1A2) reduces theophylline clearance.
What to do
Monitor theophylline serum levels and signs of toxicity during the first days of the first cycle and when restarting after an interruption. Reduce the theophylline dose if levels rise.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
