Summary
Barbiturates are enzyme inducers and can lower bosentan exposure, which may reduce its effect on pulmonary arterial pressure. Bosentan is itself an inducer, so the net change is hard to predict and clinical data with pentobarbital are limited.
What this interaction means in plain words
Bosentan and Pentobarbital can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Pentobarbital induces CYP3A4 and CYP2C9, the main enzymes that metabolize bosentan, increasing its clearance.
What to do
Monitor clinical response in pulmonary arterial hypertension (functional class, symptoms) when starting or stopping pentobarbital. Consider an alternative sedative if efficacy declines.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
