Summary
Combining formoterol with another beta-agonist such as metaproterenol adds beta-adrenergic stimulation. This can cause tachycardia, palpitations, tremor, raised blood pressure and a fall in serum potassium, with a higher arrhythmia risk in patients with heart disease.
What this interaction means in plain words
Budesonide-formoterol and Metaproterenol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive beta-2 adrenergic receptor agonism causing cardiovascular stimulation and intracellular potassium shift.
What to do
Avoid routine concurrent use of two beta-agonists; if both are needed, monitor heart rate, blood pressure and serum potassium. Rationalise to a single short-acting rescue agent alongside the maintenance inhaler.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
