Summary
Thiazide diuretics reduce urinary calcium excretion, so adding calcium supplements can raise serum calcium. Hypercalcemia and, with alkali co-ingestion, milk-alkali syndrome may occur, especially with high calcium doses or renal impairment.
What this interaction means in plain words
Calcium and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Thiazides increase distal tubular calcium reabsorption, decreasing renal calcium clearance.
What to do
Monitor serum calcium periodically, particularly when starting or increasing calcium or vitamin D. Reduce the calcium dose if hypercalcemia develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
