Summary

Valproate increases oxybate exposure, which can intensify respiratory depression, sedation and coma. Oxybates already carry serious CNS and respiratory depressant risks.

What this interaction means in plain words

Calcium / magnesium / potassium / sodium oxybates and Valproic acid can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: High

Why it happens

Valproate inhibits GHB dehydrogenase and possibly monocarboxylate transporters, reducing oxybate clearance, with additive CNS depression.

What to do

Reduce the nightly oxybate dose by at least 20% when valproate is started, or use a lower oxybate starting dose in patients already on valproate. Titrate by response and monitor for respiratory and CNS depression.

What to ask your pharmacist
