Summary

Carbapenems, including imipenem, rapidly and markedly lower serum valproate concentrations, which can lead to loss of seizure control. The effect begins within a day or two and may persist for some days after stopping the carbapenem.

What this interaction means in plain words

Cilastatin-imipenem and Valproic acid can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Cilastatin-imipenem belongs to Anti Bacterial; Valproic acid belongs to Central Nervous System.

Evidence level: High

Why it happens

Carbapenems inhibit hydrolysis of valproate glucuronide and may reduce its absorption and distribution, lowering valproate levels.

What to do

Avoid the combination and use a non-carbapenem antibiotic. Increasing the valproate dose is usually ineffective, so add or switch to another anticonvulsant if a carbapenem is unavoidable.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
