Summary

NSAIDs can blunt the antihypertensive effect of angiotensin receptor blockers. In elderly, volume-depleted or renally impaired patients the combination can cause acute kidney injury and hyperkalemia.

What this interaction means in plain words

Candesartan and Flurbiprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Candesartan belongs to Cardiovascular; Flurbiprofen belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAID inhibition of renal vasodilatory prostaglandins combined with ARB efferent arteriolar dilation reduces glomerular filtration and potassium excretion.

What to do

Maintain hydration and monitor blood pressure, serum creatinine and potassium after initiation and periodically. Avoid prolonged combination in patients with CKD or volume depletion.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
