Summary
Two ARBs together provide dual angiotensin receptor blockade, which adds little benefit but increases hyperkalemia, hypotension and renal impairment. The thiazide partly offsets potassium retention.
What this interaction means in plain words
Candesartan and Hydrochlorothiazide-telmisartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Candesartan belongs to Cardiovascular; Hydrochlorothiazide-telmisartan belongs to Cardiovascular.
Why it happens
Duplicate angiotensin II type 1 receptor blockade causing additive reduction of aldosterone effect and glomerular filtration pressure.
What to do
Avoid combining two ARBs; use a single ARB and adjust the dose instead. If concurrent use occurs, monitor potassium, creatinine and blood pressure closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
