Summary
Fenoprofen can reduce the antihypertensive effect of candesartan and hydrochlorothiazide and increase the risk of renal impairment, particularly in older or volume-depleted patients. Hyperkalemia may also occur.
What this interaction means in plain words
Candesartan-hydrochlorothiazide and Fenoprofen can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Candesartan-hydrochlorothiazide belongs to Cardiovascular; Fenoprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandins reduces renal blood flow and sodium excretion, counteracting the diuretic and ARB and adding to renal hemodynamic stress.
What to do
Avoid combining where possible, or use the lowest effective NSAID dose for a short course. Monitor blood pressure, renal function and potassium.
