Summary

Hydrochlorothiazide can raise dofetilide levels by reducing renal clearance and may cause hypokalemia or hypomagnesemia. Higher dofetilide exposure prolongs the QT interval and can provoke torsades de pointes.

What this interaction means in plain words

Candesartan-hydrochlorothiazide and Dofetilide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Candesartan-hydrochlorothiazide belongs to Cardiovascular; Dofetilide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Thiazide-related reduction in renal elimination of dofetilide, with electrolyte disturbances adding to QT prolongation.

What to do

Avoid concomitant use and select a non-thiazide antihypertensive. If unavoidable, monitor QTc, potassium and magnesium closely.
