Summary

Both the angiotensin receptor blocker and the thiazide diuretic reduce renal lithium clearance, so serum lithium can rise to toxic levels. Toxicity may present with tremor, confusion, ataxia, vomiting and, in severe cases, seizures or renal damage.

What this interaction means in plain words

Candesartan-hydrochlorothiazide and Lithium can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Candesartan-hydrochlorothiazide belongs to Cardiovascular; Lithium belongs to Central Nervous System.

Evidence level: High

Why it happens

Thiazides and RAS blockers promote sodium loss and increase proximal tubular lithium reabsorption, lowering lithium renal clearance.

What to do

Avoid if possible and choose another antihypertensive. If used, measure serum lithium before and within the first week of therapy, reduce the lithium dose as needed and monitor renal function and hydration.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
