Summary
Inhaled long-acting beta-agonists can lower serum potassium, and hydrochlorothiazide also causes potassium loss. Together they may produce hypokalemia and increase the risk of ECG changes or arrhythmia, particularly in patients with hypoxia, high doses or other risk factors. At usual inhaled doses the effect is generally modest.
What this interaction means in plain words
Candesartan-hydrochlorothiazide and Vilanterol / fluticasone furoate inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive hypokalemia from beta-2 agonist-driven intracellular potassium shift and thiazide-induced renal potassium wasting.
What to do
Check serum potassium and magnesium at baseline and periodically in patients with additional risk factors, and correct deficits. The candesartan component partly offsets potassium loss.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
