Summary
Xipamide is a thiazide-like diuretic, so adding it to a thiazide-containing combination duplicates diuretic therapy. This increases the risk of excessive blood pressure lowering, volume depletion, hypokalemia, hyponatremia and renal function decline.
What this interaction means in plain words
Candesartan-hydrochlorothiazide and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive diuretic and natriuretic effects from two thiazide-type agents acting at the distal tubule, combined with the antihypertensive effect of the angiotensin receptor blocker.
What to do
Avoid routine duplication of thiazide-type diuretics. If used together, monitor electrolytes, renal function and blood pressure closely.
What to ask your pharmacist
