Summary

Both thiazide diuretics and SSRIs can cause hyponatremia, and together the risk is increased, particularly in older adults. Thiazide-induced hypokalemia or hypomagnesemia may also add to citalopram's QT effect.

What this interaction means in plain words

Captopril-hydrochlorothiazide and Citalopram can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Captopril-hydrochlorothiazide belongs to Cardiovascular; Citalopram belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive hyponatremia via renal sodium loss (thiazide) and SIADH (SSRI).

What to do

Monitor serum sodium, particularly in the first weeks and in the elderly, and check potassium and magnesium. Stop or switch therapy if symptomatic hyponatremia develops.
