Summary
Hydrochlorothiazide can raise dofetilide concentrations and may cause hypokalemia or hypomagnesemia, both of which promote QT prolongation. This increases the risk of torsades de pointes and ventricular arrhythmias.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Dofetilide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Captopril-hydrochlorothiazide belongs to Cardiovascular; Dofetilide belongs to Cardiovascular.
Why it happens
Thiazides reduce renal clearance of dofetilide (inhibiting cationic renal secretion) and cause electrolyte depletion, adding to QT prolongation.
What to do
Avoid per dofetilide labeling; use an alternative antihypertensive or antiarrhythmic and correct potassium and magnesium if dofetilide is essential.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
