Summary

Conivaptan produces aquaresis, which may reduce circulating volume and cause hypotension in patients on an ACE inhibitor and a diuretic. Rapid rises in serum sodium are also a concern when diuretics are used together with it.

What this interaction means in plain words

Captopril-hydrochlorothiazide and Conivaptan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Vasopressin V1a/V2 antagonism causes free water loss and volume depletion, which adds to diuretic and ACE inhibitor effects on blood pressure.

What to do

Monitor blood pressure, volume status and serum sodium frequently during conivaptan infusion. Hold or reduce the antihypertensive and diuretic if hypotension or overly rapid sodium correction occurs.

What to ask your pharmacist

  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
