Summary
Cyclosporine can cause hyperkalemia, hyperuricemia and renal impairment, while thiazides can cause hyponatremia, hyperuricemia and volume depletion. Combined with an ACE inhibitor, the risks of renal dysfunction and hyperkalemia are increased, and gout risk may rise with thiazide use.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Cyclosporine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive nephrotoxic, electrolyte and hemodynamic effects of cyclosporine, the ACE inhibitor and the thiazide.
What to do
Monitor serum creatinine, potassium, sodium, uric acid and blood pressure regularly, and adjust doses if renal function or electrolytes deteriorate.
What to ask your pharmacist
