Summary
NSAIDs can blunt the blood pressure lowering of ACE inhibitors and thiazides and, together with them, increase the risk of acute kidney injury and hyperkalemia. The risk is greatest in older, volume-depleted or renally impaired patients.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Fenoprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Captopril-hydrochlorothiazide belongs to Cardiovascular; Fenoprofen belongs to Musculoskeletal.
Why it happens
Inhibition of vasodilatory renal prostaglandins reduces renal perfusion and sodium excretion, opposing antihypertensive effects while ACE inhibition and diuresis impair compensatory glomerular autoregulation.
What to do
Use the lowest effective NSAID dose for the shortest time, maintain hydration, and monitor blood pressure, creatinine and potassium. Consider paracetamol or other alternatives.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
