Summary

Heparin can suppress aldosterone secretion, and ACE inhibitors also lower aldosterone, so together they may cause hyperkalemia. Risk is greater with renal impairment, diabetes or prolonged heparin use; the thiazide partly offsets this by wasting potassium.

What this interaction means in plain words

Captopril-hydrochlorothiazide and Heparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Captopril-hydrochlorothiazide belongs to Cardiovascular; Heparin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin inhibits adrenal aldosterone synthesis, adding to reduced aldosterone from ACE inhibition and reducing renal potassium excretion.

What to do

Monitor serum potassium and renal function, particularly if heparin is given for more than a few days or in high-risk patients. Treat hyperkalemia and review the need for the ACE inhibitor if it develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
