Summary
Hydrochlorothiazide can cause potassium loss, and beta-2 agonists such as olodaterol can shift potassium into cells. Together they may produce hypokalemia, which is more likely at high doses and may predispose to arrhythmias. The captopril component partly offsets potassium loss.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Olodaterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive hypokalemia from thiazide renal potassium wasting and beta-2 mediated intracellular potassium shift.
What to do
Monitor serum potassium, especially at initiation, in patients with heart disease or at risk of arrhythmia. Replace potassium if levels are low.
What to ask your pharmacist

- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
