Summary
Bowel preparations cause fluid and electrolyte shifts and volume depletion. With a thiazide diuretic and an ACE inhibitor there is increased risk of acute kidney injury and electrolyte disturbance, including hyponatremia and hypokalemia.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Sodium sulfate / potassium chloride / magnesium sulfate / polyethylene glycol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Bowel-prep-induced fluid and electrolyte loss added to diuretic effects and ACE-inhibitor-related reduction in renal autoregulation.
What to do
Maintain adequate oral hydration and consider holding the diuretic and ACE inhibitor on the day of preparation. Check electrolytes and renal function in high-risk patients.
What to ask your pharmacist
