Summary
Vilanterol, a beta-agonist, can lower serum potassium and affect the ECG, and hydrochlorothiazide also causes potassium loss. Hypokalemia may be additive, particularly with high doses of the beta-agonist. Captopril partly offsets potassium loss.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Vilanterol / fluticasone furoate inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Beta-2 stimulated intracellular potassium shift combined with thiazide-induced renal potassium loss.
What to do
Monitor serum potassium, especially at treatment initiation and in patients with risk factors for arrhythmia. Do not exceed the recommended inhaler dose.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
