Summary

Carbachol is a direct muscarinic and nicotinic agonist that is also resistant to cholinesterase breakdown. Combined with rivastigmine, cholinergic effects may be amplified. Systemic effects are only likely with oral or injected carbachol, since ophthalmic use has little systemic exposure.

What this interaction means in plain words

Carbachol and Rivastigmine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Carbachol belongs to Miscellaneous; Rivastigmine belongs to Central Nervous System.

Evidence level: Low

Why it happens

Additive cholinergic stimulation from a direct agonist and a cholinesterase inhibitor.

What to do

Monitor for bradycardia, bronchospasm, sweating, salivation and gastrointestinal cramping. Reduce the dose of one agent if cholinergic excess develops.

What to ask your pharmacist

  • Should I take them at different times of day?
