Summary

Carbenoxolone causes sodium retention and potassium loss, and together with hydrochlorothiazide may produce marked hypokalemia and fluid retention that opposes blood pressure control. Losartan only partly offsets this.

What this interaction means in plain words

Carbenoxolone and Hydrochlorothiazide-losartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Carbenoxolone has mineralocorticoid-like activity by inhibiting 11-beta-hydroxysteroid dehydrogenase, adding to thiazide potassium wasting.

What to do

Avoid the combination if possible. If used, monitor serum potassium, blood pressure and fluid status closely.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
