Summary

Carbidopa, usually given with levodopa, increases peripheral dopamine formation from levodopa, which can lower blood pressure and cause orthostatic hypotension. Combined with carvedilol, which has vasodilating alpha-blocking action, hypotension may be more pronounced, particularly when levodopa therapy is started or increased.

What this interaction means in plain words

Carbidopa and Carvedilol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Carbidopa belongs to Central Nervous System; Carvedilol belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic hypotensive effects of levodopa-derived dopamine and carvedilol's alpha/beta blockade.

What to do

Monitor supine and standing blood pressure when starting or titrating levodopa/carbidopa. Reduce the carvedilol dose if symptomatic hypotension occurs.
