Summary
Oliceridine is metabolized by CYP3A4 and CYP2D6, so inhibitors can increase opioid exposure and respiratory depression. Entacapone is not an established CYP3A4 inhibitor, and levodopa may add sedation or hypotension. Opioids may also worsen constipation in Parkinson's disease.
What this interaction means in plain words
Carbidopa-entacapone-levodopa and Oliceridine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Carbidopa-entacapone-levodopa belongs to Central Nervous System; Oliceridine belongs to Musculoskeletal.
Why it happens
Additive CNS and hypotensive effects, with a theoretical CYP3A4 inhibition by entacapone that is not well established.
What to do
Titrate oliceridine to effect, monitoring for sedation, respiratory depression and orthostatic hypotension. Use the lowest effective dose.
