Overview

Severity: Moderate · Evidence level: Moderate

Carbimazole can induce or worsen hypothyroidism; hypothyroidism is a recognized risk factor for rosuvastatin-associated myopathy/rhabdomyolysis, increasing the likelihood of muscle toxicity during periods of low thyroid function.

Mechanism of action

Pharmacodynamic: Hypothyroidism predisposes to statin-induced myotoxicity; no direct CYP/transporter interaction is expected.

Clinical management

Ensure the patient is euthyroid before initiating or up-titrating rosuvastatin; monitor TSH every 4–6 weeks after carbimazole dose changes; counsel patients to report muscle pain/weakness or dark urine; if hypothyroidism develops or muscle symptoms occur, check CK and consider holding or reducing rosuvastatin until euthyroid status is restored.
