Summary

Oral fecal microbiota products contain live bacterial spores intended to recolonize the gut. A systemic cephalosporin that is still present may reduce the viability or engraftment of these organisms and weaken the preventive effect against recurrent C. difficile infection.

What this interaction means in plain words

Cefaclor and Microbiota can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Antibacterial activity against the live bacterial components of the product reduces colonization (pharmacodynamic antagonism).

What to do

Finish the antibiotic course and allow a washout interval of a few days before starting the microbiota product, as the labeling advises. Do not give the product during active cephalosporin therapy.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
