Summary

Concurrent NSAIDs and systemic corticosteroids increase the risk of peptic ulceration and gastrointestinal bleeding more than either alone. Celecoxib has lower GI risk than non-selective NSAIDs, but the risk is still raised with steroids. Fluid retention may also be additive.

What this interaction means in plain words

Celecoxib and Cortisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Additive impairment of gastric mucosal defense through prostaglandin inhibition (NSAID) and corticosteroid effects on mucosal healing.

What to do

Use the lowest effective doses for the shortest duration and consider a proton pump inhibitor for gastroprotection in at-risk patients. Monitor for GI bleeding symptoms, and for blood pressure and fluid retention.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
