Summary
Celecoxib can blunt the blood pressure lowering effect of olmesartan. In patients who are elderly, volume depleted or have existing renal impairment, the combination may also worsen kidney function and raise serum potassium.
What this interaction means in plain words
Celecoxib and Olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Celecoxib belongs to Musculoskeletal; Olmesartan belongs to Cardiovascular.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion, opposing angiotensin receptor blockade and impairing glomerular filtration.
What to do
Monitor blood pressure, serum creatinine and potassium after starting or changing the combination, and keep patients well hydrated. Use the lowest effective celecoxib dose for the shortest time or choose a non-NSAID analgesic in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
