Summary
Celecoxib can blunt the blood pressure lowering effect of perindopril. In patients who are elderly, volume-depleted or have renal impairment, the combination may cause acute kidney injury, usually reversible, and hyperkalemia.
What this interaction means in plain words
Celecoxib and Perindopril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Celecoxib belongs to Musculoskeletal; Perindopril belongs to Cardiovascular.
Why it happens
Inhibition of vasodilatory renal prostaglandin synthesis by the NSAID opposes ACE inhibitor effects and reduces glomerular perfusion.
What to do
Monitor blood pressure, serum creatinine and potassium after starting or changing therapy, and ensure adequate hydration. Use the lowest effective celecoxib dose for the shortest time, or choose an alternative analgesic in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
