Summary

NSAIDs such as celecoxib can reduce renal function in patients taking sacubitril/valsartan, especially the elderly, volume-depleted or those with existing kidney impairment. They can also blunt the blood pressure lowering effect and promote hyperkalaemia and fluid retention.

What this interaction means in plain words

Celecoxib and Sacubitril / valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Inhibition of vasodilatory renal prostaglandins reduces glomerular perfusion, which combines with angiotensin receptor blockade to impair renal function and potassium excretion.

What to do

Monitor serum creatinine, potassium and blood pressure after starting or changing the NSAID. Use the lowest effective NSAID dose for the shortest time, or choose an alternative analgesic in high-risk patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
