Summary
Sulfasalazine contains a sulfonamide-type moiety, and sulfonamide antibacterials are known to potentiate sulfonylurea hypoglycemia. Reports with sulfasalazine are sparse, but an enhanced glucose-lowering effect of chlorpropamide is plausible, and chlorpropamide's long half-life makes prolonged hypoglycemia a concern.
What this interaction means in plain words
Chlorpropamide and Sulphasalazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Chlorpropamide belongs to Endocrine; Sulphasalazine belongs to Gastrointestinal Tract.
Why it happens
Possible displacement from plasma protein binding and/or reduced sulfonylurea clearance; the mechanism is not well established for sulfasalazine.
What to do
Monitor blood glucose more frequently when sulfasalazine is started or stopped in patients on chlorpropamide. Reduce the sulfonylurea dose if hypoglycemia occurs and counsel the patient on its symptoms.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
