Summary

Combining chlorthalidone with a product that already contains hydrochlorothiazide duplicates thiazide-type diuretic therapy. This increases the risk of excessive diuresis, hypokalemia, hyponatremia, hypotension and renal function decline, with little added benefit.

What this interaction means in plain words

Chlorthalidone and Hydrochlorothiazide-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Chlorthalidone belongs to Cardiovascular; Hydrochlorothiazide-valsartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive inhibition of the distal tubule sodium-chloride cotransporter by two thiazide-type diuretics, increasing urinary sodium and potassium loss.

What to do

Avoid routine therapeutic duplication and use only one thiazide-type agent unless a specialist deliberately combines them. If both are used, monitor electrolytes, renal function and blood pressure closely.
