Overview
Magnesium-containing products (e.g., Epimag) chelate oral ciprofloxacin in the GI tract, substantially reducing ciprofloxacin absorption and exposure, risking treatment failure.
Mechanism of action
Chelation of ciprofloxacin by polyvalent cations (Mg2+) in the gastrointestinal tract → decreased oral absorption and serum concentrations.
Clinical management
Avoid coadministration. If necessary, administer ciprofloxacin at least 2 hours before or 6 hours after magnesium/aluminum products; monitor clinical response and consider alternative non-cationic laxatives during therapy.
