Summary
Cocaine blocks catecholamine and serotonin reuptake, and with an MAO inhibitor such as procarbazine there is risk of hypertensive crisis, arrhythmia and serotonin toxicity. Topical medical cocaine has limited absorption but the combination is still advised against.
What this interaction means in plain words
Cocaine and Procarbazine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Cocaine belongs to Central Nervous System; Procarbazine belongs to Oncology.
Why it happens
MAO inhibition increases monoamine stores while cocaine blocks reuptake, producing additive sympathomimetic and serotonergic effects.
What to do
Avoid cocaine, including topical use, during and for 2 weeks after procarbazine. Use an alternative local anesthetic.
