Summary
Voriconazole increases conivaptan exposure, which may cause excessively rapid sodium rise, hypotension and osmotic demyelination risk.
What this interaction means in plain words
Conivaptan and Voriconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Strong CYP3A4 inhibition by voriconazole decreases conivaptan metabolism.
What to do
Do not co-administer; use another hyponatremia therapy or antifungal. Monitor serum sodium closely if exposure occurs.
