Summary
Giving an anthracycline together with dactinomycin can cause radiation recall reactions, including pneumonitis, in tissue that was previously irradiated. This has been described in children and can appear at variable times after the radiotherapy. The reaction may be severe and needs prompt recognition.
What this interaction means in plain words
Dactinomycin and Doxorubicin liposomal can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Both agents are radiosensitizing and can reactivate latent radiation-induced tissue injury, causing a recall reaction.
What to do
Review prior radiation fields before starting the combination and monitor for cough, dyspnea or hypoxia. If pneumonitis is suspected, hold the drugs and evaluate for treatment with corticosteroids.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
